Registering Your Child

We thank you for considering our Faith Formation program for your child. Registration forms for new and returning students are below. Please drop off your registration form and payment in an envelope marked "Faith Formation" in the collection basket at Mass. You may also mail it in to the address at the bottom of the form, or bring it in to the church office.

REGISTRATION FOR 2026-2027

Please check the schedule located on the website of both parishes for your child's specific class time.

2026-2027 FAITH FORMATION SCHEDULE

Please note that we will need a copy of your child's baptismal certificate if your child was NOT baptized at St. Nicholas of Myra or Annunciation of the Lord parish. 

If your child was baptized at St. Nicholas of Myra or Annunciation of the Lord, please provide the date of their baptism.

If you would like to Home School your child, please check off the box on the form.
All classes are held at: Saint Nicholas of Myra Parish Hall

SIGN UP NOW! We need registration forms as soon as possible in order for us to plan for class size, materials, and catechists.

 

Classes begin on Sunday September 13^th for Grades 1-5 and Monday September 14^th for grades 6-7.   Confirmation begins on Sunday September 27th for grades 8 & 9.
